I recently obtained a copy of my credit report. Some of the information on the credit report is wrong. The specific account tradeline that is wrong is listed above. The information about this tradeline is wrong because: (choose one or more):

b. this is not my account. I suffered an identity theft and filed fraud alerts with (list Experian, TransUnion, Equifax, local police department) on (list date);
c. this is my account but the information is wrong because the debt was discharged in bankruptcy several years ago. I have attached a copy of my bankruptcy discharge for your review. The date of last activity should be the date of the bankruptcy discharge;
d. this account is too old. This account is more than 7 ½ years old and should be deleted from my account;
Enclosed is a copy of the relevant entry from my credit report so that you can accurately delete my name from any credit reporting on this account and a copy of the letter that I sent to creditor with all attachments.
